Due to on going additions in our participation with more insurance companies, we encourage that you call our office to verify our in-network participation with your insurance carrier and whether a Doctor’s referral is required by your  insurance company. You may also call your insurance company to verify your in-network copay or deductible, which is due at the time of visit. If you have any questions, our office would be glad to assist you with your physical therapy benefits.
